The Way Free Medical Clinic’s Expanding Capacity to Care Initiative
has been launched to provide “more services to more people” and finalize the organization’s local solution to a national problem: the lack of affordable healthcare to the underprivileged. This unprecedented growth initiative sets the stage to double the number of people served by the clinic in three years.
Here’s how we plan on achieving this important goal:
The Way Clinic’s Headquarters: 302 College Drive
Thanks to the lead donation of a commercial building on College Drive in Orange Park, The Way Clinic is establishing a headquarters facility in the heart of the county. This new facility will be the hub of a countywide free clinic system. We will be maintaining the Green Cove Springs clinic as a satellite, and eventually establishing other satellite services in rural areas of the county.
Located within close proximity of countless health and community services agencies, as well as all three Clay-based hospitals, the facility will dramatically increase The Way Clinic’s ability to reach more patients in need. The facility will also integrate the only free dental clinic in Clay County into its operation, bolstering the county’s lack of dental care options to low-income residents. The facility is anticipated to be 7,600 square feet and will be renovated into an infectious disease-ready facility with touchless features and high-filtration ventilation
This initial capital element of the initiative is expected to be a $1.0M project.
ClayPASS: Coordinated Medical Care Initiative
ClayPASS coordinated medical care initiative identifies and links providers through a data sharing system, navigates patients to the most appropriate care, coordinates that care, and reduces costly hospital utilization. The initiative embeds case management into all three Clay-based hospitals to divert unnecessary use of emergency departments for primary care and prevent inpatient hospital readmissions. When fully launched in 2022, this initiative will begin saving the healthcare system millions of dollars each year. In addition to clinical navigation, the project will connect patients where possible to Medicaid, Medicare, and other insurance policies to provide access those qualified to enroll.
The Way Clinic is the lead partner in the initiative and the organizations involved in planning, implementation, and operation of the network include: Ascension St. Vincent's Clay County Hospital; Baptist Clay Medical Center; Orange Park Medical Center; The Palms Medical Group; Azalea Health; Clay County Sheriff's Office Detention Center; Clay County Department of Health; Clay County Fire & Rescue; and Health Planning Council of NE FL. The project is endorsed by Impact Clay, the county’s emerging community services project incubator whose leadership includes the Presidents of each of the three Clay-based hospitals.
The operational cost will be approximately $225,000 each year and will produce an ROI (Return on Investment) of 10:1 when fully implemented.
Operating the Two Clinic System
The beginning of a countywide "hub and spoke" medical system for the underprivileged, the new College Drive clinic and the existing Green Cove Springs clinic will be run in tandem. This will immediately double medical care access points for the uninsured and provide easier access in the heart of the county. Future expansion will include extending services into the more rural areas of Clay County.
The operational cost of running the second clinic facility will be approximately $100,000 each year.
